Membership enrollment

We are so excited that you’ve decided to join the Peach Blossom family! Please read this page carefully so you understand the process. Membership enrollment involves a few steps:

1. Click the button below for the membership option you’d like

2. You will be redirected to the SigmaMD Enrollment site

   • First enter your own information and click the radio button on the bottom where it says “I am enrolling others, but not myself.”

   • Click “save,” then click “add” to enter each child’s information. If you are pre-enrolling a newborn, you can use a placeholder name and today’s date as the DOB (as it won’t let you add a DOB that is in the future)

   • Then select how often you’d like to be billed (monthly, quarterly, or annually), remembering that annual payments for the Core Membership receive a 5% discount

   • Then click “continue” and on a subsequent screen you’ll choose when you’d like the membership to start (it can start on the due date if you are pre-enrolling a newborn)

   • Then you will enter your payment information and attest to having read the membership agreement

   • You will receive an email with a registration code for the SigmaMD app - be sure to download the app and register, as this is how we’ll communicate and where you will schedule appointments, view/change billing information, and view your child’s records

   • SigmaMD will prompt you to complete their standard intake form - you do not need to do this. Instead, move on to step 3 below.

3. After your SigmaMD enrollment is completed, please complete an Intake Form for each child you are enrolling - please enter your child’s name on the login screen (even where it asks for your name)

   • New Patient Intake Form (for children 2 months of age or older)

   • Newborn Intake Form (for those still in-utero through 2 months of age)

   • Once you’ve completed the New Patient Intake form(s), you will receive an email containing the members-only phone number (for urgent after-hours calls) and website login information - please save these for your records, but do not share them with anyone
